Patient Payments



If you choose to pay for your visit out-of-pocket, Keystone offers a Prompt Pay Discount. Please call our office for more information. All appointments are one hour in length and payment will be collected prior to each visit.


Physical Therapy Supplies

If you purchase any physical therapy supplies, payment will be collected at the end of your appointment.  If you believe your insurance company will pay for the supply item, Keystone will provide you with a receipt so that you can submit the claim yourself.

Explanation of Charges

Your treatment will include a combination of the following therapies.


During the Initial evaluation (which is required by law) your therapist will ask you questions about the problems you are experiencing and become familiar with your injury, level of activity and lifestyle.  The therapist will evaluate the diagnosis your doctor has presented and establish a plan of care with you.


Therapeutic procedures or exercises are defined as procedures to develop strength, flexibility, endurance and range of motion.


Manual therapy is defined as hands on procedures such as soft tissue mobilization, joint mobilization, myofascial release and other techniques described in Our Services tab on this website.

Health Insurance


We are network providers with many of the major health insurances. Please call our clinic to confirm if we are networked with your insurance company. If we are out-of-network we will gladly contact your insurance provider to find out your benefits and then discuss payment options with you. Keystone Physical Therapy accepts Workers Compensation, Medicare and Medicare Replacement Plans.



Co-pays are due at the time of each visit.



Your coinsurance will be billed to you after Keystone receives the Explanation of Benefit (EOB) from your insurance provider.
